23歲男子一夜情不帶保險套 發燒紅疹才知梅毒上身
一名23歲年輕男性，兩個月前，和一名女網友春風一度，10天前全身紅疹發燒，就醫檢查，赫然發現是二期梅毒，醫師還建議患者同時檢驗愛滋病毒，幸好是陰性。在安全性行為衛教、並施打一劑盤尼西林後，目前已痊癒。(寇世菁報導)
台中醫院感染科醫師曾婷玉表示，這名23歲男子，因為全身紅疹發燒就醫，進一步詢問病史，患者透露，大約兩個月前曾與女網友春風一度，並未戴保險套，一個月前曾在生殖器發現一個小小潰瘍，因為不會痛，不以為意。醫師安排理學檢查發現，紅疹分布在身體與四肢，手掌與腳掌也出現紅疹。抽血檢查後，確診是二期梅毒。另外，建議患者同時檢驗愛滋病毒，幸好報告是陰性，沒有感染愛滋病。在給予安全性行為衛教、施打一劑盤尼西林後，目前痊癒。
醫師指出，梅毒在古代俗稱偉大的模仿者，症狀變異性很大，潛伏期可能長達90天。一期梅毒是指在接觸感染病患體液處形成的病灶，通常是生殖器或口腔，特色是無痛性潰瘍，這時期傳染性極高。潰瘍在沒有治療狀況下，也會在幾週後癒合，通常梅毒螺旋體已進入血液循環，並擴散到全身，成為二期梅毒，常見的症狀有發燒、皮疹、掉髮、關節痛、扁平濕疣等。感染梅毒後也可能完全沒有症狀，進入潛伏期梅毒，體內仍有梅毒螺旋菌存在。梅毒會經由性行為、輸血或母子垂直感染。當傷口接觸到感染病患的精液、陰道分泌物、血液等，就可能被傳染。愛滋病毒與梅毒的傳染途徑相似，因此通常會建議患者一併檢查。梅毒的檢驗方式主要依靠抽血，須經過兩個關卡:RPR/VDRV與TPPA/TPHA；若兩個檢驗項目均為陽性，即確診為梅毒。對於梅毒的治療，可在門診接受盤尼西林肌肉注射，若是對盤尼西林過敏，亦有口服抗生素可治療。完成抗生素療即算治癒，但仍有再感染的風險，因此須持續追蹤RPR/VDRV數值；但RPR/VDRV數值治療後下降緩慢，一般建議3-6個月之後再追蹤。醫師提醒民眾，梅毒是常見的性傳染病，傳染後可能沒有症狀，因此建議有過不安全性行為者至少篩檢一次，且建議伴侶一起抽血檢查，並同時檢查愛滋病毒。由於梅毒是經由血液、體液傳染，一般生活、共用餐具及衛浴並不會被傳染，家人不須太緊張。預防梅毒最好的方式為避免多重性伴侶，避免與性生活史不明的對象發生性行為，並全程使用保險套。若有性病相關的疑問可至感染科門診諮詢。
空污法修法綠讓步 25日順利三讀
【台灣醒報記者譚有勝台北報導】空污法25日力拚三讀，原本關鍵條文空污總量管制「會同、會商」無共識，國民黨試圖提400案臨時動議杯葛議程，最終民進黨妥協，空污法改為由環保署「會商」經濟部，但仍需報請行政院核准。國民黨也撤案以示善意，順利完成二讀，將連夜拚三讀。除了會同改為會商外，二讀條文規定，企業可透過收購老舊車輛的方式取得排放量，並訂定施行10年後將停止適用的落日條款。
立法院朝野黨團過去8度協商空污法草案，但9條關鍵條文包括空污總量管制、移動污染源抵換機制等仍未達共識，立法院院長蘇嘉全隨即宣布在下午1時30分處理爭議條文，朝野先進行廣泛討論，並針對保留條文逐條討論，最終上演表決大戰。
針對空污法第9條文，空污總量管制的規定，環保署與經濟部是「會同」或「會商」關係，朝野一直爭執不休。國民黨要求，空污總量管制應由環保署主責，主要「會商」經濟部即可。
國民黨立委王育敏批評，若空污法由環保署「會同」經濟部，未來環保署恐被經濟部架空，空污總量管制審查形同過場。但環保署長李應元認為，空污管制攸關環保與經濟，唯有透過會同才能夠讓權責分明，並沒有權責失衡的問題。
然而，國民黨在下午院會時提出400案臨時動議欲杯葛議事程序，最終藍綠兩黨協商後，民進黨讓步，將環保署「會同」經濟部改為「會商」 經濟部，但加入「報請政院分期分區核定公告實施」意即行政院擁有最終空污總量管制決定權。國民黨也妥協撤案，預計今晚9點以前能順利完成三讀。
國民黨立法院黨團書記長李彥秀表示，雖然民進黨在關鍵時刻做了修正，但卻要請行政院核准，這樣修正恐怕是在打假球。她強調，空污條文應給環保署更多權力，若環保署只要會同經濟部，最後的決定權仍在行政院手上，環保署只會被經濟部牽著鼻子走。
朝野立委大約在下午4時左右進行表決，出席91人中，56人贊成、35人反對，通過民主進步黨立法院黨團所提出的再修正動議內容。二讀條文規定，固定污染源若無法完成指定的削減排放量，應依法改善移動污染源減少的排放量，例如企業可透過收購老舊車輛的方式取得排放量，加速老車的汰換，並訂定施行10年後將停止適用的落日條款。
條文也規定，新設或變更的固定污染源、無法達成指定削減排放量目標的既存固定污染源，應自固定污染源交易或拍賣取得的排放量、改善移動污染源所減少的排放量等，取得抵換的排放量，並以較低的比例抵換。
