 3個月大男嬰吃不下 竟是惡性畸胎瘤

孩子持續腹痛、或出現嘔吐及黑便情形，照顧者別掉以輕心！一名3個月大男嬰出現腹脹、嘔吐及食慾不振，母親見狀趕緊帶其到醫院檢查，經檢查初步判定是畸胎瘤，再手術切除腫瘤，所幸未轉移，病理報告結果顯示是未成熟畸胎瘤，為惡性畸胎瘤，術後恢復良好。

替該病童診治的三軍總醫院小兒外科主治醫師林介文表示，藉由腹部超音波檢查發現患者腹部有一腫瘤病灶，住院之後安排一系列的身體檢查及影像檢查，初步判定為畸胎瘤，但來源不明。於是安排剖腹探查手術將腫瘤切除，發現腫瘤來源為胃部前壁，術前評估及術中發現未有看到淋巴及其他腹內轉移，最後病理報告結果顯示為未成熟畸胎瘤。病童術後恢復良好並於2周後順利出院。

林介文說明，畸胎瘤是生殖細胞腫瘤中常見的一種，是由內、中、外三胚層演變而來的先天性腫瘤，分為成熟畸胎瘤（即良性畸胎瘤）和未成熟性畸胎瘤（惡性畸胎瘤）。良性畸胎瘤裡含有很多種成分，包括皮膚、毛髮、牙齒、骨骼、油脂、神經組織等；惡性畸胎瘤分化欠佳，沒有或少有成形的組織，結構不清。

林介文指出，早期畸胎瘤多無明顯臨床症狀，大多是體檢時偶然發現。胃畸胎瘤是一種罕見的腫瘤，在兒童畸胎瘤中發生率小於1%，絕大多數為良性，惡性者較少見，目前國內外發表的病例約為三十多例。術前常被誤診為神經母細胞瘤、胰臟囊腫、網膜及腸系膜囊腫、脾囊腫或腎母細胞瘤。

因此病缺乏典型的臨床表現，早期診斷困難。 林介文說明，臨床主要表現為腹部腫塊，多位於上腹，腹脹，嘔吐，嘔血和（或）黑便，呼吸困難，貧血。在新生兒或嬰兒中，上腹腫塊及上消化道出血（以間歇性黑便多見）為主要臨床表現。胃畸胎瘤多為良性，及早手術完全切除，預後良好，不需要額外的化學治療。術後應長期追蹤，定期檢查AFP(胎兒球蛋白)，看有無復發或轉移。

林介文提醒，現代社會家庭繁忙，為人父母或照顧者需要多注意家中小孩的狀況，若有持續腹脹腹痛及食慾不振等狀況，甚至有嘔吐及黑便的狀況，更應提高警覺，就近帶至小兒科診所或是醫院，經由詳細的身體檢查及病史詢問，得到正確的診斷及治療，只要及時治療，預後相對也會比較好。
